EMS Cardiac Arrest Program

The Maryland Cardiac Arrest Public Defibrillation (M-CAPD) Study will ascertain the incidence and location of out-of-hospital sudden cardiac arrest. Data will be collected on all cardiac arrests (except those with a valid EMS DNR at scene or those DOA) from several sources:

The purpose of the Supplemental EMS Cardiac Arrest Form (available in PDF format for viewing or downloading) is to collect data necessary to enable analysis by jurisdictions and MIEMSS to:

  • Establish EMS System effectiveness in responding to Cardiac Arrest patients;
  • Improve patient outcome from cardiac arrest.

The current Supplemental EMS Cardiac Arrest Form and Instructions are dated November 2000. The Supplemental EMS Cardiac Arrest Form, MAIS, and AED summary report should be faxed to MIEMSS Epidemiology immediately following the incident. [Fax: (410) 706-4366 or toll-free at (877) 787-8089]
