Prescription Pad for Your Child’s Safety

The CPS & OP Healthcare Project will send you FREE copies of the
Prescription Pad for Your Child’s Safety for use by healthcare providers
educating parents and caregivers about the proper use of child restraints.

To request copies of the Prescription Pad for use in your office or clinic,

Note: the Prescription Pad is available in English and Spanish!

On the reverse side of the Prescription Pad, you will find the 5-Step Test for helping
parents to determine if their school-aged child should still be riding in a booster.
